Here’s how to add your vaccination information to your digital wallet:

  1. Obtain a downloadable file or QR code from your vaccination provider. If you are not sure how to do this or if you have, contact your insurance company, or pharmacy or the place where you have been vaccinated.
  2. If you do not know where you got your vaccine from, or where your doses have been given at different locations, you can access your Illinois vaccination record here.
  3. Make sure your iPhone is also updated with the iOS 15.1 software.
  4. If you have a QR code: Open the Camera app on your phone and place it on top of the code. When the Health app notification appears, tap and select “Add to Vault and Health”
  5. If you have a downloadable file: drag it to your phone and tap on the download link. Then, select “Add to wallet and health.”

Contact your healthcare provider.

If you lose or damage your Vaccine Card (or if you have never received one before) the first step is to contact the healthcare provider who administered the shot.

Many companies, including CVS and Walgreen’s, will keep a record of these vaccinations and can help individuals get new cards.

Other providers, including hospitals that run large-scale vaccination clinics, can also help.

Contact the Illinois State Department of Health.

If you are unable to contact your healthcare provider, or where you received the vaccination dose, the state health department should be able to assist through their immunization information system.

The law requires vaccine providers to report IV and other systems to the COVID-19 vaccine. Illinois records can be accessed. Through this portal.

According to the CDC, these programs, which are designed to provide information about how individuals have performed after receiving their vaccines, also provide vaccine information to those individuals. Will be the ones who signed up.

Here’s more about V-Safe, and more about VaxText.
